Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the needs of children and their families.

• Participate in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring alignment with each child’s unique developmental trajectory.

• Monitor and document the progress of children while actively participating as a member of a multidisciplinary team.
\n
• Foster collaborative relationships with families, providing guidance and practical support tailored to enhance their child’s growth.

• Adhere to all relevant federal and state regulations while delivering client-centered services.

• Implement evidence-based practices and natural learning environment strategies during home visits and family sessions.

• Engage in ongoing professional development and training to stay current with best practices in early intervention and social work.

Patient Population / Client Focus\n

You will primarily work with infants and toddlers diagnosed with disabilities or developmental delays, as well as their families. Your efforts will focus on helping these children develop essential skills across various domains, including physical, cognitive, social/emotional, and communication skills. By delivering supportive and therapeutic services, you will contribute significantly to improving long-term outcomes for these young individuals.

Qualifications\n

• A Master's Degree in Social Work or a Doctorate of Social Work from an accredited institution.

• A current Licensed Clinical Social Worker (LCSW) credential, approved by the Department of Public Health.

• A valid driver's license for travel to family homes, community facilities, and other relevant sites.

• Excellent organizational skills, with a proven ability to manage time effectively in a fast-paced environment.

Compensation and Benefits\n
