Key Responsibilities\n

• Conduct comprehensive assessments to understand and document the needs of the patients you serve, tailoring interventions accordingly.

• Collaborate effectively with supervisors, direct care team members, and external stakeholders, including family members and guardians, to track and communicate client progress.
\n
• Develop and maintain a tentative schedule of appointments and home visits, ensuring thorough monitoring of each client’s developmental journey.
\n
• Oversee home visits to gather critical insights through direct observations and chart reviews, ensuring that care aligns with established therapeutic goals.
\n
• Implement and monitor the effectiveness of individualized program plans, adjusting strategies as needed to enhance client outcomes.
\n
• Participate actively in interdisciplinary team meetings to discuss client progress and refine treatment strategies.
\n
• Document progress through monthly and quarterly notes, maintaining accurate and readily available records.
\n
• Act as a liaison with the broader mental health community, ensuring alignment with development goals and facilitating support networks for clients.

• Assist in staff training programs to enhance team competency in the application of program plans and documentation standards.
\n
• Provide ongoing support through follow-up interviews and home visits, ensuring continued alignment with each patient’s unique needs.

In this role, you will work primarily with diverse individuals, including both adults and youth, from various backgrounds navigating challenges related to mental health. Our patient population may present with issues ranging from anxiety and depression to trauma and substance use disorders. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted license as a Professional Counselor (LPC) in the state of Texas.

• A minimum of two years’ counseling experience in a clinical context, along with proven success in engaging with clients to facilitate recovery.

• Strong understanding of legal and ethical standards relevant to counseling practices.

• Excellent interpersonal and communication skills, with the ability to build rapport with a diverse clientele.

• Demonstrated ability to collaborate effectively within a multidisciplinary healthcare team.

• Proficiency in EMR software and other relevant technology platforms used in delivering client care.

Preferred Experience\n
