Work Environment

This position requires an onsite presence as the Physical Therapist will work directly within patients' homes. Key Responsibilities

As a Licensed Physical Therapist with Ouvunius Rehabilitation Services, your essential duties will include:

• Conducting comprehensive patient evaluations to assess functional abilities and identify rehabilitation needs. Document these evaluations according to established clinical guidelines and standards.
• Creating, implementing, and modifying personalized treatment plans to facilitate optimal patient outcomes, employing evidence-based practices specific to individual needs.
• Regularly documenting patient progress through accurate progress notes and reassessments, ensuring compliance with applicable guidelines and organizational policies.
• Administering effective physical therapy treatments that support patients in achieving their rehabilitation goals, continually adapting methods to maximize effectiveness based on patient progress.
• Collaborating with interdisciplinary teams, relatives, and caregivers to provide holistic care and ensure that all stakeholders are informed of treatment plans and progress.

Patient Population / Client Focus

This role primarily involves working with a diverse patient population in their homes. You will encounter individuals of varying age groups, including pediatrics, adults, and seniors, each with distinctive rehabilitation needs. This flexibility requires you to adapt your techniques and communication strategies to effectively engage and motivate patients throughout their recovery process.

Qualifications

The successful candidate will meet the following qualifications:

• Graduated from an accredited Doctorate in Physical Therapy (DPT) program.
• Possession of an active and unrestricted Physical Therapist license in the state of California.
• A clean professional history, free from any sanctions or disciplinary actions.
• Demonstrated proficiency in a range of manual therapy techniques and rehabilitation practices.

Preferred Experience

While not mandatory, candidates with experience in home health settings or working with geriatric populations would be favored. Familiarity with rehabilitative technologies and telehealth services will also be an asset.
