Work Environment

This position is fully onsite, requiring visits to patients' homes throughout the Sacramento area. You will work in diverse and varying home environments, allowing you to assess and adapt your therapies to meet individual patient needs effectively. The schedule is structured for Monday to Friday, from 9 AM to 5 PM, allowing for work-life balance while ensuring that you can devote ample time to each patient.

Key Responsibilities

• Conduct thorough physical therapy evaluations to develop tailored treatment plans based on individual patient assessments.
• Utilize evidence-based practices to administer therapeutic exercises and interventions that promote mobility and functional independence.
• Educate patients and their families on exercise techniques, injury prevention, and self-care strategies that will aid in recovery.
• Monitor and document patient progress, noting changes in condition and adapting treatment plans as necessary to ensure optimal outcomes.
• Collaborate with healthcare professionals, including physicians, nurses, and social workers, to coordinate comprehensive patient care.
• Adhere to all established safety protocols and maintain an infection control environment to protect both patients and staff.
• Participate in ongoing training and professional development opportunities to stay current with industry best practices.

Patient Population / Client Focus

You will work primarily with a diverse clientele, including elderly adults recovering from surgeries, individuals with chronic pain, and patients needing rehabilitation after injury. Your role will be crucial in facilitating recovery and helping clients regain their mobility, strength, and confidence. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A current and valid physical therapy license in the state of California.
• A minimum of two years of experience in a clinical setting, preferably with home care experience.

Preferred Experience

• Experience working with geriatric patients and those requiring post-surgical rehabilitation.
• Familiarity with home care settings and the challenges associated with delivering therapy in a patient's own environment.

Compensation and Benefits

The compensation for this full-time Physical Therapist role ranges from $110,000 to $125,000 annually, depending on experience.
