Work Environment

As a Physical Therapist at our Rehabilitation Center, you will work in an onsite setting designed to foster collaboration and communication among healthcare professionals. Our facility is equipped with the latest technology and tools to facilitate effective rehabilitation processes, creating a welcoming space for both patients and staff. The work schedule is Monday through Friday from 8:00 AM to 5:00 PM, promoting a balanced work-life dynamic.

Key Responsibilities

• Evaluate and assess patients' physical conditions through initial consultations, developing individualized treatment plans to address functional limitations and disabilities.
• Implement evidence-based treatment protocols, adjusting methodologies as necessary based on ongoing evaluations and patient feedback.
• Document patient care activities accurately and timely, including conducting initial evaluations, daily progress notes, and discharge summaries that comply with evolving healthcare standards.
• Collaborate with interdisciplinary teams of healthcare professionals to ensure cohesive and comprehensive patient care.
• Recognize changes in patients' conditions that necessitate a modification in treatment plans, proactively addressing any emerging issues.
• Participate in discharge planning and follow-up care, ensuring a smooth transition for patients from therapy to independent management.
• Stay up to date with state practice regulations and ensure compliance across all clinical directives and corporate training requirements.
• Acknowledge barriers to healthcare access and engage in initiatives aimed at improving service delivery.
• Attend facility meetings and contribute to discussions on best practices and enhancements within our rehabilitation services.

Patient Population / Client Focus

Your primary focus will be on outpatient orthopedic patients who require physical therapy interventions post-injury, surgery, or due to chronic conditions. You will work closely with individuals ranging from athletes aiming to return to sport, to seniors seeking improved mobility, making your impact significant across various demographics within the community.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program is required.
• A valid and active physical therapist license in the state of New Jersey.
• A current certification in CPR to ensure safety and preparedness in any given situation.

Preferred Experience

• A minimum of two years of clinical experience in outpatient orthopedic settings is preferred.
• Familiarity with electronic medical record (EMR) systems to streamline documentation and patient management.
• A background in collaborative treatment approaches integrating physical therapy with other healthcare disciplines.
