Key Responsibilities\n

• Conduct comprehensive assessments and evaluations to identify patients’ psychosocial needs, developing and recommending tailored treatment plans.

• Provide direct counseling and therapeutic interventions through evidence-based practices, effectively managing a diverse case load.

• Assist patients in navigating and completing various application forms for entitlements, insurance, and supplemental benefits, ensuring they receive necessary supports.
\n
• Advocate for clients when interfacing with community organizations and services, enhancing access to essential resources and support.
\n
• Maintain meticulous and up-to-date electronic health records, ensuring compliance with agency regulations and patient confidentiality.
\n
• Coordinate transportation solutions as required to facilitate patients' attendance at appointments and treatment sessions.
\n
• Participate in regular case conferences and team meetings, contributing insights and feedback to improve patient care processes.
\n
• Assess and identify the need for educational materials and support resources for both patients and their families.
\n
• Implement assessments and provide educational guidance for prenatal and postpartum patients, ensuring comprehensive care.
\n
• Collaborate closely with providers for necessary referrals, aligning treatment strategies with comprehensive health goals.
\n
• Evaluate and monitor the needs of specific populations, including those requiring tailored intervention strategies.
\n
• Conduct HIV pre-test and post-test counseling, providing essential information and support.
\n
• Follow up on missed appointments to re-engage patients and address barriers to care.
\n
• Engage in quality improvement and assurance initiatives to enhance service delivery and patient outcomes.

• Maintain a tidy and professional remote working environment, reflecting commitment to our organizational values.

• Perform other relevant duties as required to meet departmental needs and patient care objectives.

Qualifications\n

• A Bachelor’s degree in Social Work or a related field is required.

• A valid Licensed Mental Health Counselor (LMHC) credential is preferred, enabling a higher level of practice and responsibility.

• At least two years of professional experience as a Social Worker is required; five or more years is preferred for this role.

• Strong interpersonal, oral, and written communication skills are essential for effective patient engagement and collaboration with multidisciplinary teams.

• Proficiency in MS Office Suite (Word, Excel, Outlook) is essential for documentation, reporting, and communication purposes.

Preferred Experience\n

Compensation and Benefits\n

Compass Clinical Services offers a competitive annual salary ranging from USD 115,000 to USD 130,000.
