Key Responsibilities\n

• Act as a liaison for clients, families, and outpatient providers, facilitating seamless communication and care coordination.

• Engage with insurance companies to obtain pre-certifications and manage utilization effectively, ensuring clients receive necessary support.

• Participate actively in discharge and aftercare planning to provide a continuous support system for clients transitioning out of detox.
\n
• Offer therapeutic meal support, demonstrating healthy relationships with food to promote both emotional and physical well-being.

• Deliver safe, empathetic, and affirming care that is responsive to each client's intersecting identities, ensuring culturally competent support tailored to individual circumstances.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals in detoxification, a critical phase in their recovery journey. Our clients may come from a variety of backgrounds and have diverse needs, including those struggling with substance use disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current, valid LCSW license in Pennsylvania.

Compensation and Benefits\n
