Key Responsibilities\n

• Conduct comprehensive assessments, including clinical interviews and standardized assessments, to accurately evaluate patient needs and determine individualized treatment plans.

• Maintain clear and consistent communication with supervisors, clinical staff, and patients’ families to discuss progress, challenges, and adjustments to treatment plans.

• Prepare a structured schedule for patient appointments, in-home visits, and necessary observations to ensure systematic monitoring and follow-up on patient care.

• Organize and execute home visits as required, documenting observations and gathering data to inform treatment modifications and outcomes.

• Oversee the implementation of care plans, ensuring that therapeutic goals are achieved effectively and efficiently.

• Participate actively in interdisciplinary team meetings, contributing insights and collaborating on strategies to enhance patient care.

• Deliver monthly and quarterly progress notes, ensuring compliance and clarity in documentation practices.

• Coordinate training for new staff members, ensuring competency in the implementation of care protocols and documentation standards.

• Offer ongoing support to patients through follow-up interviews and additional home visits where necessary, addressing evolving needs.

• Serve as a liaison between patients, families, and community resources, aiding in the connection to external support systems as needed.

Qualifications\n
