Key Responsibilities\n

• Engage directly with assigned clients facing severe emotional disturbances and major mental illnesses, conducting thorough assessments to develop appropriate treatment plans.

• Provide a comprehensive range of brief, focused prevention, treatment, and recovery services tailored to each client's specific needs and circumstances.

• Participate actively in multidisciplinary care team meetings, sharing insights and contributing to collaborative decision-making regarding client care.

• Facilitate goal-oriented discussions with patients and their families, ensuring alignment with their needs and advancing care plans effectively.

• Serve as a patient advocate, effectively balancing the healthcare business needs with the urgent requirement for compassionate client care.

• Build and maintain strong relationships with internal team members, matrix partners, and healthcare providers, ensuring coordinated care strategies.

Qualifications\n

• Minimum of 3 years of professional experience in social work, particularly with clients facing severe emotional and mental health challenges.
