Key Responsibilities\n

• Apply advanced knowledge and training to formulate independent psychosocial diagnoses, guiding treatment decisions based on patient needs and circumstances.

• Collaborate with patients to create tailored treatment plans that reflect their personal goals, preferences, and values, ensuring patient-centered care.

• Utilize evidence-based therapeutic interventions, including solution-focused treatments, behavioral activation, motivitional interviewing, and problem-solving therapy.

• Coordinate with a multidisciplinary team including psychologists, primary care providers, and other clinicians to create an integrated approach to each patient’s care.
\n
• Track the efficacy of treatment interventions and assess patient progress towards therapeutic goals through regular monitoring, either virtually or in person, and make adjustments to treatment plans as necessary.

• Assist in managing psychotropic medication prescriptions, focusing on adherence and monitoring for side effects and overall treatment effectiveness.

Patient Population / Client Focus\n

In this role, you will primarily engage with individuals coping with serious illnesses in a hospice setting. Patients may present with a range of psychological and emotional challenges, including anticipatory grief, anxiety, and depression. Supporting families during this time is also a vital part of the role, as you provide resources and therapy to enhance their coping mechanisms.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in the state of New York.

• A minimum of one year of experience as an independently licensed therapist within a mental health or healthcare environment.

Preferred Experience\n

• Experience working in hospice care or with terminally ill patients is highly desirable.

• Familiarity with telehealth technology and frameworks for delivering virtual therapy is advantageous.
