Housing Case Manager
The People Concern
The People Concern is hiring an ICMS Case Manager to support clients in achieving housing stability and self-sufficiency through evidence-based practices such as harm reduction, housing first, and trauma-informed care in a 100% onsite setting. You will manage a caseload, collaborate with LA County DMH and DHS, and coordinate services to meet individualized needs. Strong drivers include independence, community integration, and ongoing support for recently housed clients. #J-18808-Ljbffr
