Key Responsibilities\n

As a Telehealth Licensed Professional Counselor, you will:

• Conduct ongoing assessments to identify and meet the diverse mental health needs of patients, ensuring personalized care plans.

• Engage in regular communication with supervisors, healthcare team members, family members, and other relevant parties to provide updates on patient progress and treatment adjustments.
\n
• Provide scheduled appointments, home visits, and observations to monitor progress and ensure adherence to treatment plans.
\n
• Coordinate in-home visits and assessments, integrating data analysis and chart reviews for comprehensive evaluations.
\n
• Implement effective program plans by closely monitoring therapeutic interventions and patient responses, adjusting as necessary.
\n
• Participate and contribute actively in interdisciplinary team meetings to discuss treatment strategies and progress.
\n
• Document comprehensive monthly and quarterly progress notes to track patient outcomes and care effectiveness.
\n
• Develop and deliver training sessions to ensure staff competence in program implementation and documentation procedures.
\n
• Maintain thorough and accurate records to facilitate easy access to patient information for authorized individuals.

• Act as a liaison within the public and mental health communities, supporting collaborative development goals.

• Assist with creating training materials to improve ongoing program efficacy for our client population.

Patient Population / Client Focus\n

You will work with a varied clientele, including adults and youth facing challenges such as anxiety, depression, trauma, and substance use. Your role will be pivotal in delivering compassionate care and personalized treatment approaches that cater to the unique needs of each individual, fostering their journey towards mental wellness.

Qualifications\n

To be a successful candidate, you must possess:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Arizona.

• A minimum of two years of counseling experience, preferably within a clinical context.

• A solid understanding of legal and ethical standards in mental health practice.

• Exceptional interpersonal and communication skills, with an ability to connect and empathize with clients from diverse backgrounds.

Preferred Experience\n

While not mandatory, candidates with the following experience will be given preference:

Compensation and Benefits\n
