Work Environment\n

This position is based remotely, providing flexibility while ensuring that you remain connected to a supportive team of health professionals. Working hours are Monday through Friday from 9:00 AM to 5:00 PM, allowing for a structured yet adaptable work schedule. Key Responsibilities\n

• Provide individualized psychotherapy and mental health services to patients experiencing conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).

• Conduct comprehensive assessments of patients’ mental health status, including evaluations of their emotional, behavioral, and social functioning.
\n
• Develop and implement tailored treatment plans based on evidence-based practices, ensuring cultural competence and sensitivity to each patient’s unique circumstances.

• Collaborate with other healthcare professionals, including psychiatrists and primary care physicians, to provide coordinated and effective care to patients.
\n
• Monitor and evaluate patient progress regularly, making adjustments to treatment plans based on patient feedback, clinical outcomes, and interdisciplinary insights.

• Educate and support patients and their families, providing them with valuable resources related to mental health conditions and community services.
\n
• Uphold ethical and professional standards, including maintaining patient confidentiality and obtaining informed consent.

Patient Population / Client Focus\n

You will work closely with diverse patient populations, including individuals facing challenging mental health issues associated with terminal illnesses or severe emotional distress. This role is pivotal in addressing the emotional well-being of patients within a hospice setting, where mental health is often intertwined with medical care. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Texas.

• A minimum of 3 years of experience providing psychotherapy and mental health services, with a focus on geriatric or hospice populations preferred.

• Strong interpersonal and communication skills to foster trusting relationships with patients and collaborate effectively with a multidisciplinary team.

• Proficient in conducting thorough patient assessments and developing effective, evidence-based treatment approaches.

• Experience with electronic health records (EHRs) and familiarity with related healthcare software for managing patient documentation and care coordination.

• A deep understanding of various mental health disorders and an empathetic approach to the complexities faced by patients dealing with these conditions.

Preferred Experience\n

• Experience in hospice, palliative care, or acute healthcare environments is highly desirable.

• Expertise in different therapeutic modalities, such as Cognitive Behavioral Therapy (CBT), and trauma-informed care is a plus.

• Familiarity with community resources and support systems available to patients and families in distress.

• Proficiency in Spanish or another language is an advantage.

Compensation and Benefits\n
