Key Responsibilities\n

• Conduct comprehensive clinical evaluations to assess clients’ psychosocial needs and develop individualized treatment plans that promote successful transitions post-discharge.

• Provide counseling sessions, utilizing evidence-based therapeutic techniques tailored to the unique circumstances of each client while ensuring ongoing monitoring of medication and potential side effects under the supervision of a Psychiatrist.

• Document all interactions and care decisions diligently, including biopsychosocial assessments, treatment goals, progress notes, and discharge planning in compliance with company policies and state regulations.

• Participate actively in multidisciplinary treatment team meetings and case management discussions, advocating for clients’ needs and ensuring continuity in care.

• Research, refer, and advocate for community resources that can improve clients' well-being, including access to food assistance, childcare, health care services, and other essential resources.

Patient Population / Client Focus\n

This role focuses on clients within a hospice setting, many of whom are experiencing end-of-life transitions. As a Clinical Social Worker, you will support patients and their families, addressing emotional, psychological, and social challenges associated with serious illness. Sutter Health Counseling, LLC offers competitive compensation with an annual salary range of $120,000 to $130,000, commensurate with experience.
