Key Responsibilities\n

• Conduct clinical social work evaluations to assess clients' psychological and emotional well-being, and develop tailored treatment plans.

• Monitor clients' medication side effects in collaboration with psychiatrists, ensuring safety and efficacy in treatment.

• Develop aftercare plans that enable clients to successfully transition from intensive care settings to independent living.

• Document and maintain case records in compliance with company policies, including biopsychosocial assessments and progress notes.

• Participate actively in case management and care coordination, contributing to treatment team meetings and utilization reviews.

• Implement crisis intervention strategies to effectively address clients in immediate need of support.

• Research and provide referrals for community resources, such as housing, healthcare, and social services to promote client well-being.

This role primarily serves individuals dealing with complex health challenges, particularly those at end-of-life stages, as well as their families. Qualifications\n

• A Master's Degree in Social Work from an accredited institution is required.
