We are currently seeking a highly qualified PT to join our growing healthcare team for part-time work, offering pay from $65 to $85 per visit, and flexible schedules. We provide the highest quality of home care in the Denver, Elbert, Arapahoe, Adams, Douglas, Jefferson, Gilpin, El Paso, Boulder, Larimer, Clear Creek, Weld area.

Key Responsibilities:

• Conduct initial and ongoing physical therapy assessments.

• Perform accurate OASIS collection

• Develop and implement individualized therapy care plans.

• Document assessments, treatments, and progress in accordance with agency and state requirements.

• Provide therapeutic exercises, gait training, balance activities, and mobility support.

• Educate clients and caregivers on exercises, equipment use, and safety techniques.

• Monitor progress, adjust treatment plans, and document outcomes.

• Collaborate with physicians, nurses, and other care team members.

Qualifications:

• Current and valid Physical Therapist (PT) license in Colorado.

• Degree in Physical Therapy from an accredited program.

• Home care, rehabilitation, or out client experience preferred.

• Strong communication and teaching skills to engage clients and families.

• Strong clinical, communication, and problem-solving skills.

• Ability to work independently in varied home environments.

• Reliable transportation and a valid driver’s license, and automobile insurance for home visits.
