Work Environment

This role will be performed onsite at our state-of-the-art Assisted Living Facility located in the vibrant city of San Antonio, Texas. As a Lead Nurse Practitioner, you will be working within a multidisciplinary team dedicated to fostering a collaborative environment. Your work schedule will typically consist of 12-hour shifts, totaling approximately 36 to 40 hours per week, allowing for a balance between your professional responsibilities and personal life.

Key Responsibilities

• Conduct comprehensive assessments of residents, diagnosing both acute and chronic conditions to initiate appropriate care plans.
• Collaborate closely with physicians, nurses, and allied health professionals to develop and execute personalized, evidence-based treatment plans.
• Utilize advanced diagnostic tools and methods to order, perform, and interpret diagnostic tests, ensuring timely interventions.
• Prescribe medications, therapies, and lifestyle modifications tailored to individual resident needs, emphasizing medication adherence and effective management strategies.
• Engage residents and their families in educational moments, guiding them through disease management, care transitions, and post-discharge needs to promote a higher quality of life.
• Participate regularly in interdisciplinary rounds, sharing insights and strategies to enhance patient outcomes and establish seamless care continuity.
• Maintain accurate and comprehensive electronic medical records, adhering to regulatory compliance and organizational standards.
• Contribute to quality improvement initiatives by integrating evidence-based practice guidelines into daily routines and staff training.
• Stay informed of the latest advancements in healthcare policies, medical device technologies, and treatment methodologies.

Patient Population / Client Focus

Our Assisted Living Facility is home to a diverse population of elderly residents, many of whom face chronic medical complexities and varying degrees of mobility and cognitive challenges. As a Lead Nurse Practitioner, your role will involve ensuring that each resident's care plan addresses their unique health status, preferences, and cultural values, with particular attention to fostering dignity and autonomy.

Qualifications

• A current Nurse Practitioner certification.
• An active nursing license in the state of Texas.
• A Bachelor's degree in nursing or a related field; a Master's degree in nursing is preferred.
• A minimum of 3 years of clinical experience in a nurse practitioner role, preferably within an assisted living or long-term care setting.
• Exceptional communication and interpersonal skills, with a demonstrated ability to work effectively in a collaborative environment.

Preferred Experience

Compensation and Benefits

Gotham Enterprises Ltd offers a competitive salary ranging from USD 120,000 to USD 220,000 per year, which is commensurate with experience and qualifications.
