Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand and address the multifaceted needs of patients transitioning into hospice care.

• Facilitate individual, family, and group therapy sessions tailored to the specific emotional and psychological challenges faced by those in hospice.

• Utilize evidence-based approaches such as cognitive behavioral therapy (CBT) and mindfulness-based interventions to support clients during this critical phase of their lives.

• Develop and implement customized treatment plans that address client-specific goals and desired outcomes concerning end-of-life care.

• Work collaboratively with physicians, nurses, and other specialists to ensure a unified approach to each patient's care plan.

• Provide crisis intervention support and risk assessments when necessary, ensuring clients feel safe and supported throughout their care.

• Maintain accurate, timely, and confidential documentation adhering to ethical standards and legal requirements.

• Organize and lead educational workshops on coping strategies for families and caregivers dealing with grief and loss.

Patient Population / Client Focus\n

The primary client focus of this role includes patients suffering from terminal illnesses, as well as their families. You will provide essential support that addresses psychological distress stemming from grief, loss, and the emotional challenges related to end-of-life issues. Qualifications\n

• A Master's degree in Social Work from a nationally accredited program.

• Possession of an active Licensed Independent Clinical Social Worker (LICSW) license in the state of Massachusetts.

• At least two years of experience working in a mental health or outpatient setting, preferably with hospice or palliative care experience.

Compensation and Benefits\n
