Key Responsibilities\n

As a Licensed Professional Counselor at Frontier Behavioral & Mental Health Clinic, your responsibilities will include:

• Conducting comprehensive patient intakes, focusing on diagnostic evaluations, and advising the primary care team on treatment goals and planning.

• Providing brief, evidence-based interventions to patients requiring mental health services, utilizing proven therapeutic techniques.

• Collaborating with primary care providers to discern which patients require ongoing mental health support and guiding those in need towards appropriate specialty care.

• Following up with patients regularly to assess their progress, adapt treatment plans as necessary, and develop relapse prevention strategies.

• Identifying and monitoring patients at risk for deterioration of their mental health, creating preventative measures that support stabilization and recovery.

• Documenting patient progress meticulously to maintain accurate treatment charts that comply with agency standards and enhance continuity of care.

• Offering consultation to management and team members about the integration of behavioral health services and potential areas for program improvement.

• Assisting patients in adhering to prescribed medical treatments and supporting them through potential medication side effects by offering coping strategies.
