The role is set within our Hospice Center, which focuses on providing compassionate care for patients with terminal illnesses and supporting their families during challenging times.

Key Responsibilities\n

• Directly engage with clients in their homes and within the community, addressing their unique mental health needs and symptoms of mental illness.

• Collaborate effectively with team members to further collective goals, ensuring a cohesive approach to client care.

• Establish trusting relationships with clients by creating a supportive environment that respects diverse backgrounds and experiences.

• Conduct evidence-based screenings and assessments to gather crucial information about clients’ mental health and wellness.

• Develop, implement, and monitor integrated care plans, linking clients to necessary services and ensuring continuity of care.

• Provide brief intervention services, including crisis management when needed, to support clients during critical periods.
\n
• Maintain accurate and timely documentation related to patient assessments, treatment plans, and progress notes.
\n
• Conduct individual, group, and family therapy sessions as part of a comprehensive treatment strategy.

• Serve on committees and quality improvement initiatives to enhance service delivery and patient outcomes.

• Provide on-call assistance as required, ensuring continuous support for patients beyond regular hours.

Patient Population / Client Focus\n

The ideal candidate will work primarily with individuals facing severe emotional disorders and major mental health challenges, a large portion of whom come from economically disadvantaged backgrounds. You must be prepared to engage with clients experiencing emotional distress and trauma, providing support that addresses both their immediate needs and long-term wellness goals.

Qualifications\n

• A Master’s Degree in Social Work (MSW) is required, with a current Illinois LCSW license.

Compensation and Benefits\n

This role comes with a competitive salary range of USD 70,000 to 85,000 per year, contingent upon experience and qualifications.
