Key Responsibilities\n

• Document and oversee transitions between hospital and inpatient services, ensuring seamless communication with healthcare professionals to support successful discharges.

• Conduct assessments, develop personalized treatment plans, and provide individual counseling sessions tailored to the needs of each participant.

Patient Population / Client Focus\n

Qualifications\n

Compensation and Benefits\n
