Key Responsibilities\n

As a Licensed Professional Counselor, you will have the following responsibilities:

• Conduct comprehensive assessments to identify the mental health needs of patients and craft customized care plans.

• Maintain clear communication with supervisors, direct care staff, families, and other stakeholders regarding patient progress.

• Prepare and submit tentative schedules for patient appointments, home visits, and observation sessions to monitor treatment effectiveness.

• Coordinate and conduct home visits to evaluate patients in their natural environments through in-home observations, chart reviews, and staff interviews.

• Ensure the effective implementation of treatment plans and monitor their execution regularly.
\n
• Engage in Interdisciplinary Team Meetings to discuss treatment plans and collaborate on patient care strategies.

• Document monthly and quarterly progress notes that reflect patient development and adherence to treatment goals.
\n
• Organize and facilitate training for staff on the implementation of treatment plans and documentation practices.
\n
• Conduct follow-up interviews and home visits to provide ongoing support for patients’ evolving needs.

• Serve as a liaison to the broader mental health community, contributing to public outreach and support initiatives.

• Assist in the development of training materials and procedures to enhance program efficacy.

Qualifications\n

The ideal candidate for this role will possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

Compensation and Benefits\n
