Key Responsibilities\n

In your role as an LCSW, you will report directly to the Social Work Manager and will be responsible for the following:

• Conducting comprehensive bio-psychosocial assessments to identify psychosocial problems and barriers to treatment, tailoring your approach based on the individual needs of patients and their families.

• Developing and implementing individualized treatment plans, utilizing both individual and group therapies grounded in evidence-based methodologies.

• Providing timely documentation of social work interventions in patients’ medical records to ensure continuity and coordination of care.

• Collaborating with healthcare professionals to deliver holistic care and improve treatment compliance, patient satisfaction, and overall health outcomes.

• Facilitating patient and family education to enhance understanding and adherence to medical recommendations.

• Staying informed on community resources to better advocate for patients' needs and facilitate access to necessary services.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in end-of-life care settings, focusing on addressing complex psychosocial issues that impact their well-being. Your ability to empathize with patients and their families, who may be experiencing grief, anxiety, and various socio-environmental challenges, will be critical to their care experience. Qualifications\n
