Key Responsibilities\n

As a Licensed Clinical Professional Counselor, you will:

• Conduct comprehensive patient intake assessments that include diagnostic evaluations and functional analyses to inform treatment plans.

• Develop and implement focused therapeutic interventions tailored to individual patient needs, ensuring adherence to Agency and Federally Qualified Health Center (FQHC) guidelines.

• Collaborate effectively with primary care providers, providing insights about mental health treatment goals and recommendations for specialty referrals when necessary.
\n
• Monitor patient progress regularly, initiating follow-up consultations to track treatment effectiveness and make necessary adjustments.
\n
• Provide education and resources for relapse prevention and self-management, empowering patients to take an active role in their recovery.
\n
• Document all clinical interactions meticulously in patient treatment charts, ensuring compliance with organizational documentation standards.
\n
• Assist in identifying at-risk patients and developing proactive plans to mitigate the escalation of mental health issues.
\n
• Offer behavioral health insights to clinic management and team members to enhance program evaluation and service delivery.
\n
• Support patients in adhering to medical directives from primary care providers, including management of any medication-related side effects.

• Uphold the Code of Ethical Conduct for Clinical Professionals and carry out additional duties as assigned.

Patient Population / Client Focus\n

Our clinic serves a diverse client base from various demographics, including individuals, couples, and families facing mental health challenges. You will work with patients experiencing a wide range of issues, including anxiety, depression, relationship difficulties, and psychosocial stressors. Qualifications\n

Candidates for this position must have:

• A minimum of a Master’s Degree in Clinical Counseling.

• A current and valid Licensed Clinical Professional Counselor (LCPC) License.

• Strong knowledge of developmental psychopathology and family therapy principles.

• Ability to assess patients’ needs and evaluate care effectively.

Preferred Experience\n

Bi-lingual proficiency in English and Spanish is preferred, as it enhances our ability to serve a broader community. Candidates should also have knowledge of community resources and behavioral health services. Previous experience in a telehealth setting is advantageous but not mandatory.
