Key Responsibilities\n

• Conduct comprehensive assessments to evaluate patient needs and develop tailored treatment plans that are actionable and effective.

• Maintain open lines of communication with supervisors, client care teams, family members, and other stakeholders regarding progress and challenges in patient care.

• Provide regular updates and a detailed schedule of counseling appointments, home visits for assessment, and ongoing observations to ensure accountability and transparency.

• Coordinate in-home observations and interactions, integrating chart reviews and data analysis to enhance the effectiveness of treatment plans.

• Ensure that program plans are implemented effectively by monitoring treatment execution and making necessary adjustments based on client progress.

• Participate actively in interdisciplinary team meetings, bringing insights and feedback to enhance collaborative care efforts.

• Document monthly and quarterly patient progress notes meticulously to track advancements and identify service adjustments.
\n
• Organize training sessions for staff to ensure competence in delivering program plans and adhering to documentation standards.

• Conduct follow-ups through home visits and interviews to provide ongoing support based on individual client needs.

• Serve as a liaison with the mental health community, assisting in the development of community outreach initiatives.

• Assist in crafting and updating training materials aimed at enhancing program efficacy for individual clients.

In your role as an LPC, you will work with diverse client populations, including adults and youth. Your focus will encompass various mental health challenges, including trauma, substance use disorders, family and couples therapy, and more. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

• A minimum of 2 years of experience in counseling, ideally within a clinical context.

• A strong understanding of legal and ethical standards in mental health care.
