Work Environment

This full-time position is primarily onsite, as you will be traveling to patients' homes throughout New Jersey to provide therapy services. Our team works Monday through Friday, with standard hours from 9 AM to 5 PM, allowing for a balanced work-life dynamic. In this role, you will leverage electronic medical records (EMR) solutions to document patient progress and treatment plans effectively while collaborating with diverse healthcare professionals.

Key Responsibilities

• Deliver thorough and evidence-based orthopedic therapy to patients in the comfort of their homes, ensuring the highest standard of care and safety.
• Engage in collaborative planning with in-house healthcare providers, including MDs, OTs, and DCs, to formulate and implement integrated care strategies tailored to individual patient needs.
• Conduct comprehensive assessments of patients' mobility, safety, and functional abilities within their home environments, allowing for tailored therapeutic interventions.
• Educate patients and their families on effective exercises, body mechanics, and adaptive techniques, empowering them to take an active role in their recovery process.
• Utilize secure, user-friendly EMR tools to document treatment sessions, maintaining accurate records of patient progress and outcomes.
• Participate in interdisciplinary case reviews and ongoing clinical education to enhance professional development and improve patient care practices.

Patient Population / Client Focus

The Home Health Physical Therapist will work primarily with patients recovering from orthopedic surgery, experiencing acute or chronic musculoskeletal conditions, and those in need of rehabilitation due to sports injuries or other physical challenges. Your work will be pivotal in promoting functional independence and improving patients' quality of life right where they feel most comfortable—their homes.

Qualifications

• Active Physical Therapist license in the State of New Jersey, or eligibility for licensure.
• Bachelor's Degree in Physical Therapy from an accredited program.
• Minimum of 2 years of experience as a PT with a focus in orthopedic outpatient settings or homecare.
• Strong interpersonal skills and a compassionate approach to patient interaction.
• Ability to work independently, demonstrating flexibility and competence in managing a homecare caseload.
• A passion for personalized, patient-centered care and a commitment to continuous learning.

Preferred Experience

• Homecare or outpatient orthopedic experience strongly preferred, though confident new graduates with relevant internships or clinical rotations may be considered.
• Proven organizational skills to manage multiple patient schedules and therapy plans effectively.

Compensation and Benefits
