Key Responsibilities\n

• Provide empathetic support to patients and families navigating behavioral health crises, ensuring they feel heard and validated throughout their experience.

• Manage case referrals by assessing individual needs, identifying care gaps, and implementing follow-up plans to facilitate progress and determine case closure.

• Guide patients and families through their recovery processes by offering psychoeducation, providing them with tailored resources and solutions.

• Evaluate social determinants of health, including housing instability, income challenges, food insecurity, and access to transportation, to inform comprehensive care strategies.

• Connect clients with relevant support groups and community networks that bolster mental wellness and provide ongoing encouragement.

• Work closely with interdisciplinary team members to create and maintain treatment plans that align with the individual goals of patients and their families.

• Regularly participate in structured consultations with team members to review patient progress, share insights, and refine care strategies.

Patient Population / Client Focus\n

You will primarily engage with families and individuals facing significant behavioral and medical complexity, including those dealing with acute mental health issues such as suicidality, aggression, and severe persistent mental illness. Your role will involve understanding the unique challenges faced by these populations and working with them to navigate their paths toward recovery.

Qualifications\n

• A Master's degree in Social Work (MSW) is required to provide the level of care necessary in this role.

• An active and valid license to practice as an LCSW in California is mandatory.

The remuneration for this position is competitive, with an annual salary ranging from $110,000 to $130,000, complemented by a robust benefits package.
