Key Responsibilities\n

• Effectively communicate patient progress and concerns to the healthcare team, including physicians and care coordinators, ensuring a comprehensive understanding of each patient's needs.

• Maintain accurate and timely documentation of clinical records and progress notes in alignment with departmental policies and standards to support optimal treatment planning.

• Collaborate with multidisciplinary teams to develop and implement individualized treatment plans based on psychosocial assessments and accepted therapeutic interventions.

• Provide individualized counseling and crisis intervention services, assessing and addressing emotional and behavioral issues with patients in both routine and emergency situations.

• Assist clients with discharge planning by coordinating care with various community providers and resources to facilitate seamless transitions and continued support.

• Engage with the clients' family members or support systems to collaborate on treatment and planning, ensuring a holistic approach to patient care.

Patient Population / Client Focus\n

As a member of our team, you will work with a diverse client population, including individuals facing a wide array of mental health challenges, such as anxiety, depression, trauma, and stress-related disorders. Qualifications\n

• A Master’s degree in Professional Counseling or a closely related field from an accredited program is required.

• Current licensure as a Licensed Professional Counselor (LPC) or equivalent credential is mandatory.

Compensation and Benefits\n
