Key Responsibilities\n

• Provide supportive counseling services tailored to individual patient needs while adhering to your clinical training and state license.

• Conduct educational and goal-directed counseling sessions for patients seeking transplantation, assessing their psychological readiness and emotional needs.
\n
• Identify and connect patients with community resources, such as home health services and vocational rehabilitation, to enhance their treatment outcomes.
\n
• Stay current with local vocational and educational opportunities to assist patients in accessing resources that support employment and educational aspirations.
\n
• Evaluate patients’ awareness of advance directives, facilitating discussions and assisting with the completion of necessary documentation in coordination with the healthcare team and patient families.

• Assess patients’ psychosocial status as part of an interdisciplinary team to address their strengths and needs, contributing towards comprehensive care assessments.
\n
• Collaborate with patients and healthcare professionals in care planning to design effective interventions aimed at meeting rehabilitation, treatment goals, and improving overall quality of life.

• Utilize established social work methodologies and quality-of-life measurement tools in assessments to identify and address barriers to achieving patient treatment goals.
\n
• Provide ongoing support and interventions to assist patients in adjusting to chronic health conditions, ensuring they maintain optimal psychosocial status and quality of life.

Patient Population / Client Focus\n

In this role, you will engage with a diverse patient population, including individuals facing significant life transitions, chronic illness, and mental health challenges. By leveraging your skills in empathy and active listening, you will create a safe and supportive environment for clients to express their concerns and work towards recovery.

Qualifications\n

• Master’s degree in Psychology, Counseling, Social Work, or a related discipline.
\n
• Current licensure as a mental health counselor (LMHC, LMFT, or LICSW) in accordance with state regulations.

• No history of medical malpractice claims as verified by a third party.
\n
• Authorization to work in the United States.
\n

Preferred Experience\n

• Experience in telehealth counseling or virtual therapy is highly desirable.

• Previous experience providing counseling to patients dealing with chronic illnesses or navigating complex health care systems.

Compensation and Benefits\n
