Key Responsibilities\n

• Conduct thorough assessments of clients to understand their emotional, psychological, and social needs, establishing a baseline from which to create individualized treatment plans.

• Implement and manage comprehensive support packages designed to assist clients in overcoming obstacles and dependencies, ensuring each tailored plan aligns with measurable goals.

• Provide crisis intervention as needed, effectively managing high-stress situations to ensure client safety and well-being.

• Schedule regular follow-up sessions with clients, tracking their progress and modifying treatment strategies as necessary to promote ongoing recovery.

• Document all client interactions and treatments accurately in our electronic medical records system, ensuring compliance with all regulatory and organizational standards.

• Collaborate with other healthcare professionals in case conferences, sharing insights and strategies to optimize care and support for clients in distress.

• Educate clients about available community resources and services, facilitating referrals to support their recovery journey.

• Serve as a key member of the multidisciplinary team, working closely with other social workers, psychologists, and healthcare providers to ensure seamless service delivery.
