Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to the unique needs of each patient, considering mental health, substance abuse, and medical conditions in a collaborative primary care environment.

• Provide age and culturally informed behavioral health services, facilitating individual and group therapy sessions that address issues ranging from anxiety to complex family dynamics.

• Document clinical interactions meticulously, utilizing electronic health records to track progress notes, counseling sessions, and crisis interventions, ensuring continuity of care.

• Serve as a vital link between patients and community resources by offering referrals to local agencies, medication assistance programs, and other essential services.

• Engage with Quality Improvement teams to gather and analyze data, aiming to enhance service delivery and promote better health outcomes across the organization.

Qualifications\n

• Valid and active Licensed Clinical Social Worker (LCSW) license in the state of New York.

• A master's degree in Social Work.

• A minimum of three years of post-licensure clinical experience in a similar role, demonstrating skills in crisis intervention and counseling.

• Comprehensive understanding of therapeutic techniques, assessment methodologies, and treatment planning.

Compensation and Benefits\n
