Key Responsibilities

• Perform patient evaluations in the home care environment, assessing recovery from injury, surgery, or disease.
• Develop comprehensive treatment plans based on individual patient assessments, utilizing various therapeutic modalities such as manual therapy, therapeutic exercises, and modalities to reduce pain.
• Implement and adjust treatment interventions to promote optimal outcomes and improvements in patient functionality and mobility.
• Document patient progress, including both performance and response to therapy, using our electronic record-keeping system.
• Regularly review and update treatment goals in collaboration with the patient, caregivers, and other healthcare professionals to ensure alignment with recovery objectives.
• Provide education and support to patients, families, and other healthcare team members regarding the use of assistive devices and proper therapy techniques.
• Engage in ongoing professional development by attending workshops and participating in relevant professional organizations to stay current with best practices in physical therapy.

Patient Population / Client Focus

Your work will focus primarily on helping patients who are dealing with various conditions that affect mobility and physical function, including those recovering from orthopedic surgeries, neurological disorders, and sports injuries. Our client base ranges from pediatric to geriatric patients, each requiring a unique approach. The goal is to ensure that every patient achieves their highest potential through targeted therapy and education.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid state license to practice physical therapy in New Jersey.
• At least two years of experience in a clinical setting.
• Strong communication skills, both verbal and written, paired with proficiency in computer applications for documentation.
• Ability to adapt to varying patient needs with a flexible and agile approach.

Preferred Experience

• Experience in home health physical therapy is highly valued but not mandatory.
• Familiarity with electronic health records (EHR) systems and related software.
• Ability to engage and mentor new graduates or interns is a plus.

Compensation and Benefits
