Work Environment

This position is onsite, requiring travel to various patient residences throughout the Sanford area. Our home care model allows therapists to work closely with patients in their everyday environments, fostering greater patient engagement and applying therapeutic techniques in real-life settings. Key Responsibilities

• Collaborate with the Director of Therapy Operations (DTO) to identify and implement clinical improvement opportunities that enhance patient outcomes.
• Build and maintain strong relationships with employers, referral sources, and payer groups through consistent engagement and communication.
• Utilize clinical systems effectively to ensure accurate documentation, coding, and charge capture, ensuring that all patient care orders are followed through promptly.
• Adhere to all state and federal regulations, including the Florida State Practice Act and Workers' Compensation regulations, to maintain compliance in all facets of service delivery.
• Drive patient satisfaction by assessing feedback and developing strategies that optimize the overall client experience.
• Consistently apply best practices as per the Physical Therapy Guidebook to ensure high standards of patient care and treatment efficacy.

The ideal candidate will work with a diverse patient population, including those recovering from surgeries, injuries, or chronic conditions. Your expertise will assist in creating individualized care plans that address specific needs, supporting improvements in mobility, strength, and overall physical health. Qualifications

• Active Florida State licensure as a Physical Therapist is required.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy is needed.
• Demonstrated excellent communication, interpersonal, and organizational skills for building rapport with patients and collaboration with healthcare teammates.
• A proven ability to thrive in a fast-paced, dynamic environment, managing multiple patient needs effectively.
• Strong problem-solving abilities and critical thinking skills essential for developing customized treatment plans.

Preferred Experience

• Experience in home health physical therapy or outpatient therapy settings preferred.
• Familiarity with electronic health record (EHR) systems to support documentation and workflow management.
