Transformative Rehab & Physical Therapy is seeking a motivated and compassionate Physical Therapist to join our team in Carteret, New Jersey. This full-time position will be dedicated to delivering high-quality care to clients in a home care setting, working directly with patients to enhance their mobility and overall quality of life. Work Environment

This position is based onsite, specifically in the homes of our clients, allowing you to provide services in a familiar and comfortable environment for those you assist. Our therapists work Monday to Friday from 9 AM to 5 PM, enabling a balanced work-life schedule and allowing for consistency in patient care. As a home care therapist, you will be responsible for managing your own patient schedule while ensuring comprehensive and compassionate treatment.

Key Responsibilities

• Conduct thorough assessments and evaluations to develop individualized treatment plans tailored to the unique needs of each patient.
• Implement rehabilitation programs focused on improving patients' mobility, strength, and overall functional abilities.
• Monitor progress and assess the effectiveness of treatments, making necessary adjustments based on patient feedback and clinical observations.
• Maintain meticulous and accurate documentation including evaluations, treatment plans, progress notes, and billing information in compliance with legal standards.
• Educate patients and their families about treatment plans and support techniques to encourage active participation in their recovery process.
• Collaborate with other healthcare professionals such as physicians, occupational therapists, and nurses to deliver comprehensive care.

Patient Population / Client Focus

Our clientele primarily includes individuals recovering from orthopedic surgeries, sports injuries, and those dealing with chronic pain or movement disorders. We focus on providing tailored solutions that cater to the specific challenges faced by our diverse patient population, ensuring that each individual receives the attention and care required for effective rehabilitation.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid license to practice as a Physical Therapist in the state of New Jersey.
• A minimum of 2 years of experience providing physical therapy services, preferably in outpatient orthopedic settings.
• Clear professional record with no disciplinary actions or license infractions.
• Proficiency in manual therapy techniques and evidence-based practices relevant to patient rehabilitation.
• Strong communication skills and a demonstrated ability to work collaboratively within a healthcare team.

Preferred Experience

Experience working specifically in home care environments is highly preferred, as well as familiarity with electronic health record (EHR) systems for documentation purposes. Additional certifications in specialized therapeutic techniques will be considered an asset.

Compensation and Benefits

Transformative Rehab & Physical Therapy offers a highly competitive salary in the range of USD 100,000 to 120,000 annually, commensurate with experience. We also provide a comprehensive benefits package that includes:
