Key Responsibilities\n

• Collaborate as a vital member of the primary care team by assisting in the identification and management of mental health and psychosocial issues.

• Conduct comprehensive patient intakes, including diagnostic and functional evaluations, to inform treatment goals and intervention plans.

• Provide brief and focused interventions for patients requiring mental health support, ensuring adherence to established Agency and Federally Qualified Health Center (FQHC) guidelines.

• Advise primary care providers on patient treatment plans, identifying cases that need referrals to specialized mental health services.

• Initiate follow-up consultations to monitor patient progress and adjust treatment approaches as necessary.

• Develop personalized relapse prevention plans and aid patients in maintaining stable functioning.
\n
• Document patient interactions and progress within treatment charts, ensuring compliance with clinical standards.

• Foster healthy lifestyle changes by working with patients who experience physical disorders alongside mental health challenges.

• Assist patients in complying with medical treatments prescribed by primary care providers.

• Engage in program evaluation and outcome measurement, offering insights for behavioral health services improvement.

Patient Population / Client Focus\n

You will work with a diverse clientele, including individuals facing varying mental health challenges, psychosocial stressors, and co-occurring physical health conditions. Qualifications\n

• A minimum of a Master's Degree in Clinical Counseling or a closely related field.

• Active Licensed Professional Counselor (LPC) license in New Jersey.

• Strong understanding of DSM-IV diagnoses and various therapeutic modalities, including family therapy and group therapy.

Compensation and Benefits\n

St. Charles Counseling Services offers a competitive salary ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
