Referral Coordinator

The Referral Coordinator – Residential supports inpatient and residential treatment programs by coordinating referrals, prior authorizations (including medication and continued stay authorizations), utilization management activities, and care transitions across levels of care. This role supports individuals receiving behavioral health and substance use disorder services and works closely with treatment teams to ensure continuity of care.

Hooper Detox provides both 24/7 inpatient and outpatient withdrawal management and stabilization services for people seeking treatment for substance use disorders. Many of the staff at Hooper are in recovery themselves and have experienced the challenges that come with withdrawal and early stabilization. They serve as reminders that recovery is possible and often provide the first rays of hope for the long journey ahead.

Schedule: Monday - Friday, 7:30am - 4:00pm (Saturday & Sunday Off)

Location: Hooper Detox Center (1535 N Williams Ave. Portland, OR 97227)

Compensation: $25.05 - $33.24/hr

* Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity. External candidates: Offer max $29.07/hr per our Union contract.

MINIMUM QUALIFICATIONS:

• High School Diploma or GED AND 1 year of experience in referrals or outreach in medical setting
• Must have current CPR certification prior to start.
• Must have current Adult First Aid Certification prior to start date.
• Bi-lingual speaker of Spanish preferred.
• Knowledge of substance use disorder treatment systems preferred.

• Exemplary skills in Microsoft Office suite, including Excel, preferred.
• Ability to independently track information pursuant to individual clients and their navigation through the continuum of care.
• Must pass a pre-employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Manage inpatient and outpatient referrals for services.
• Initiate prior authorizations for medication through insurance companies.
• Track and report data related to patient program admission, progress, discharge, transition and other criteria as needed.
• Coordinate Utilization Management activities.
• Support care team and clients by participating in triage and evaluation of client needs.
• Authorization for Care: Check benefits and collect and submit clinical information necessary to submit prior authorizations and continued stay authorizations for care.
• Care Coordination and Communication: Coordinate services with all members of the care team and the client’s support network. Work collaboratively with pharmacies, insurance companies, medical providers, and behavioral health and primary care teams both internally within CCC and externally with community providers. Assist with patient care by problem-solving, eliminating unnecessary barriers to accessing services, scheduling appointments, and checking Pre-Manage.
• Documentation: Referral Specialists document Care Plans in EPIC for all patients.
• Work within scope of training and experience and seek consultation when appropriate.
• Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

• Perform other duties as assigned.
