Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to develop individualized treatment plans in collaboration with multi-disciplinary team members.

• Communicate essential patient progress updates, needs, and concerns to physicians and other healthcare providers to ensure continuity of care.

• Maintain accurate clinical documentation and progress notes in adherence to department policies and compliance standards.
\n
• Counsel patients facing emotional and behavioral challenges, employing evidence-based strategies and crisis intervention techniques as necessary.

• Engage in proactive discharge planning by coordinating with other providers and community services, ensuring effective transitions for clients.
\n
• Involve families and other support systems in the treatment and discharge planning process, enhancing overall client support.

Patient Population / Client Focus\n

Qualifications\n

• A Master's degree in Professional Counseling from an accredited institution is required.
