Key Responsibilities\n

• Provide emotional support and guidance to patients and their families who are navigating crises related to behavioral health.

• Conduct thorough case assessments to identify the family’s needs, resourcing gaps, and formulate actionable care plans.

• Facilitate psychoeducational sessions that empower families to understand and manage individual recovery goals effectively.

• Identify and address social determinants of health affecting patient care, including housing instability, financial constraints, food insecurity, and transportation challenges.

• Establish connections between patients, families, and available community resources such as support groups and mental health networks.

• Work collaboratively with colleagues across various disciplines, integrating diverse perspectives into family treatment plans.

• Participate in regular team consultations to evaluate ongoing patient progress and adjust care approaches as needed.

Patient Population / Client Focus\n

The LCSW will work primarily with individuals who are dealing with significant behavioral health issues, including those experiencing suicidal ideation, severe mental illness, or crisis situations. Additionally, your support will extend to their families, helping them understand their loved ones’ conditions and fostering an environment conducive to recovery.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is required.

• A valid and active license to practice social work in California.

• Expertise in managing cases for children, adolescents, and families within outpatient and telehealth settings.

Compensation and Benefits\n
