Key Responsibilities\n

• Conduct comprehensive assessments and create treatment plans tailored to individual needs. Engage with patients to develop personalized prevention and crisis management strategies, including behavioral guidelines and medication treatment plans.

• Document patient interactions and care plans in a timely and precise manner, adhering to all regulatory and compliance requirements.

• Utilize evidence-based screening tools alongside the clinical team to evaluate patients and inform therapeutic approaches.

• Collaborate closely with interdisciplinary teams to design and implement positive behavior support plans that address the unique circumstances of each patient.

• Serve as a liaison to various service systems, community partners, and state agencies, ensuring effective communication and coordination.

• Participate actively in licensing and accreditation processes to uphold the highest standards of quality in mental health care.

• Engage in outreach efforts to establish and maintain relationships with local, state, federal, and private organizations, identifying opportunities for partnerships and funding.

Patient Population / Client Focus\n
