Key Responsibilities\n

• Educate patients and their families about community resources and assist with post-rehabilitation planning to ensure smooth transitions back to home or alternative care settings.

• Participate in comprehensive discharge planning that focuses on the patient’s continuing care needs and successful reintegration into the community.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience, plus the potential for a sign-on bonus.
