Key Responsibilities\n

• Conduct ongoing assessments tailored to meet the individual needs of each patient, ensuring their progress is continually monitored and evaluated.

• Communicate effectively with supervisors, direct care staff, family members, and guardians regarding patient developments and treatment progress.

• Provide a tentative schedule of appointments, home visits, and clinical observations, crucial for monitoring treatment outcomes.

• Coordinate necessary home visits and assessments, including chart reviews and data analysis, to ensure comprehensive understanding of each patient's circumstances.

• Implement and oversee the execution of program plans to guarantee consistent and quality service delivery.
\n
• Participate in Interdisciplinary Team Meetings, contributing insights and collaborating on patient care strategies to enhance overall service effectiveness.

• Document and report on patient progress through detailed monthly and quarterly notes to maintain accurate records.
\n
• Organize and deliver training for staff to ensure competence in the program plan and proper documentation practices.
\n
• Provide ongoing support through follow-up interviews and home visits, ensuring that treatment goals remain aligned with patient needs.

• Serve as a liaison with the broader mental health community, building relationships to support individual development goals.

• Assist in creating procedural documentation to improve program efficiency and effectiveness for both staff and clients.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is mandatory.
