Key Responsibilities\n

• Conduct comprehensive assessments and provide clinical consultation to fellow staff on various mental health issues, ensuring appropriate intervention strategies are employed for diverse patient populations.

• Communicate critical patient information to clinical staff and interdisciplinary teams, facilitating a cohesive approach to patient care and treatment, including conveying social histories and family dynamics.

• Coordinate the development, implementation, and ongoing review of individualized patient care plans, particularly in relation to the patient's home environment, to ensure tailored support and facilitate effective treatment outcomes.

• Actively engage in treatment planning discussions, reviewing cases with other providers to ensure alignment in therapeutic approaches and to adapt strategies as necessary based on patient progress.

• Maintain accurate and timely documentation of client progress and therapy sessions to track treatment efficacy and contribute to ongoing clinical evaluations.

• Participate in team meetings and collaborate with other health professionals to enhance the overall quality of care provided.

• Adhere to all ethical standards and legal requirements pertaining to mental health practice.

Patient Population / Client Focus\n

Your role will entail working with diverse patient populations, including geriatric clients and individuals facing substance use challenges. Qualifications\n

• A Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Possession of one of the following independent licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A strong familiarity with electronic health record (EHR) systems to efficiently complete progress notes and manage documentation.

• Exceptional interpersonal skills to establish rapport with clients and collaborate effectively with team members.

Preferred Experience\n

Experience in remote therapy and using telehealth platforms will be advantageous. Prior experience working with geriatric clients and individuals facing substance use disorders is preferred but not mandatory.

Compensation and Benefits\n
