Full-time hours are typically Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs.

Key Responsibilities\n

• Conduct detailed psychosocial assessments of seniors, carefully considering their emotional, mental, and social factors that could impact their well-being.

• Develop individualized care plans in collaboration with clients and their families to reflect specific goals, preferences, and available resources.

• Provide counseling and emotional support, guiding seniors and families through difficult transitions, end-of-life decisions, and other life challenges.

• Advocate for clients' rights and ensure they have access to critical community resources, including healthcare services, support groups, and social programs.

• Regularly visit clients to assess living conditions, safety concerns, and overall well-being while maintaining accurate documentation of interactions and progress.

Compensation and Benefits\n
