Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives.
- Provide PT evaluations, develop plans of care, implement therapeutic interventions, and monitor progress for home-based patients.
- Document patient progress, outcomes, and communications with the care team in accordance with policy and regulatory requirements.
- Collaborate as a co-leader in an interdisciplinary approach to patient groups, promoting coordinated care and discharge planning.
- Educate patients and caregivers on home exercise programs, safety, and activities of daily living to support functional gains and fall prevention.
- Participate in ongoing quality improvement and ensure consistent adherence to care standards in a dynamic home health environment.

Requirements
- Active Physical Therapist license in California (state of placement) and eligibility to practice in the assigned setting.
- Ability to work 40 hours per week on daytime shifts.
- Experience in or familiarity with home health or patient-home-based PT is preferred.
- Strong documentation, organization, and interpersonal communication skills for collaboration with an interdisciplinary team.
- Willingness to participate in a travel assignment serving multiple patient locations within the defined geographic area.
