Work Environment\n

This position is remote, providing flexibility in your work schedule while you maintain a full-time commitment. You will be a part of an outpatient office model, which allows for dynamic interactions with clients and the team through telehealth and remote counseling. Similar to an in-person setting, you will engage in regular meetings with colleagues and supervisors to collaborate on patient care and therapeutic approaches.

Key Responsibilities\n

• Conduct thorough assessments of patients’ mental health needs, establishing appropriate treatment plans tailored to individual goals and challenges.

• Maintain open lines of communication with supervisors, colleagues, clients' family members, and guardians to monitor and discuss individual progress.
\n
• Create a structured schedule for client appointments, home visits, and follow-up observations, ensuring timely support and care deliveries.
\n
• Coordinate and conduct home visits as necessary for comprehensive assessments, utilizing chart reviews, interviews, and data analysis to inform treatment plans.

• Participate in interdisciplinary team meetings to discuss client progress and any adjustments required for effective program implementation.
\n
• Document monthly and quarterly patient progress and therapeutic outcomes, ensuring all records are current and accessible.
\n
• Lead training initiatives to ensure all team members are proficient in the program’s methodologies and documentation procedures.
\n
• Offer ongoing support through follow-up interviews and home visits to continually assess and respond to individual patient needs.
\n
• Act as a liaison with community services and stakeholders to foster partnerships that enhance patient care and achieve developmental objectives.

• Contribute to the development and refinement of training materials and procedures to enhance team capabilities and program effectiveness.

Patient Population / Client Focus\n

In this role, you will serve a diverse population, including both adults and youth, who are facing various mental health challenges. The center focuses on offering tailored services, addressing issues such as anxiety, depression, trauma, and relationships, with the aim of promoting holistic wellbeing.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Possession of an active and unrestricted Professional Clinical Counselor (LPCC) license in the state of California.
\n
• A minimum of two years of clinical counseling experience, demonstrating practical knowledge and skills in mental health services.

• Strong expertise in family therapy, couples counseling, trauma-informed care, or substance misuse interventions is highly regarded.

• Thorough understanding of relevant legal and ethical standards governing mental health practice.
\n
• Exceptional interpersonal and communication abilities to create meaningful relationships with clients from varied backgrounds.

• Prior experience collaborating effectively within a healthcare team environment.
\n
• Competency with Electronic Medical Records (EMR) software and other digital tools used for documentation and communication.

Preferred Experience\n
