Key Responsibilities\n

• Conduct comprehensive clinical evaluations and assessments to identify patients' needs, particularly assessing suicidality and risks of harm to self or others.

• Begin discharge planning from the day of a patient's admission, ensuring that the transition process is smooth and well-structured.

• Communicate effectively with the Utilization Review and Business Department regarding authorization processes and payment concerns, maintaining a seamless organizational workflow.

• Ensure that all documentation, including initial assessments and clinical case notes, is completed accurately and submitted promptly in accordance with our electronic health record systems.

• Confirm discharge appointments with patients and their families to ensure clarity on the transition plan and resources available post-discharge.
\n
• Collaborate with outpatient providers and referral sources to ensure continuity of care throughout the treatment process.
\n
• Assist in crisis intervention during therapy sessions and in the milieu, providing critical support to patients and advising staff as necessary.
\n
• Engage in clinical outreach and actively participate in program tours to promote our services and patient care philosophy.
\n
• Provide guidance and support to milieu staff in their interactions with patients, enhancing the overall therapeutic environment.

• Complete additional tasks as requested, adapting to the dynamic needs of the clinical team.
