Work Environment

Our Hempstead facility is an outpatient clinic designed to foster collaboration among healthcare professionals, patients, and their families. The physical therapist will work in a bright, modern setting equipped with state-of-the-art rehabilitation equipment and technology. Key Responsibilities

• Conduct thorough examinations and evaluations of patients presenting with functional limitations and disabilities.
• Develop comprehensive treatment plans tailored to each patient's needs, incorporating evidence-based practices to support recovery.
• Engage in ongoing assessment and re-evaluation of patient progress, modifying treatment protocols as necessary to enhance outcomes.
• Provide insights and recommendations to interdisciplinary teams and other healthcare providers regarding the patient's continuum of care.
• Maintain effective communication with patients and their families, ensuring they are informed and involved in the treatment process and discharge planning.
• Document all patient interactions, evaluations, treatments, and charges accurately and in compliance with all applicable regulations and organizational standards.
• Participate in leadership functions, including community outreach initiatives, program development, and continual quality improvement efforts.
• Supervise and guide assigned support staff, fostering a collaborative learning environment.
• Attend staff meetings and pursue ongoing professional development through workshops and continuing education opportunities to enhance clinical skills.

Patient Population / Client Focus

Qualifications

• Basic Life Support (BLS) certification is required for this role.

Compensation and Benefits
