Key Responsibilities\n

• Perform thorough mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments, to determine client needs accurately.

• Collaborate with patients and their families to construct individualized treatment plans that reflect personal goals, medical history, and cultural background.
\n
• Implement evidence-based therapeutic methods, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), across diverse settings such as individual, couples, family, and group therapy.

• Provide crisis intervention services and ensure that continuity of care is maintained through collaboration with multidisciplinary healthcare teams.

• Document clinical progress comprehensively, including treatment notes and discharge summaries, to meet the highest standards of confidentiality and regulatory compliance.

• Educate patients and their families by providing psychoeducational resources that empower them to manage mental health challenges effectively.
\n
• Engage with other healthcare specialists, such as psychiatrists and case managers, to coordinate holistic care strategies tailored to patient needs.

• Regularly assess treatment outcomes and make data-driven adjustments to enhance the efficacy of therapeutic interventions.

• Stay current with advancements in mental health research and best practices, applying knowledge to improve patient care.

• Uphold ethical standards in practice, ensuring the confidentiality and integrity of all patient information.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in a hospice setting, many of whom are facing end-of-life challenges. You will be integral in offering emotional support, helping families navigate grief, and empowering patients to communicate their needs effectively. Your clinical skills will help provide comfort and compassionate care during one of the most critical times in their lives.

Qualifications\n

• A Master’s degree in Social Work is required, along with an active State license as a Licensed Clinical Social Worker (LCSW).

Preferred Experience\n

Compensation and Benefits\n
