Work Environment

This position is based in the comfort of clients' homes in Fairfax, offering a one-on-one therapy setting that allows for personalized care and interaction. The role operates primarily during standard business hours, Monday through Friday from 9 AM to 5 PM, allowing for a healthy work-life balance. Key Responsibilities

• Conduct comprehensive assessments at the outset and throughout therapy to evaluate residents' mobility, pain levels, and overall functional capabilities.
• Design and implement patient-centered treatment plans tailored to the unique needs and goals of each resident, in collaboration with interdisciplinary teams.
• Provide evidence-based therapeutic interventions designed to improve strength, endurance, and flexibility—essential for enhancing mobility and minimizing pain.
• Instruct residents, families, and caregivers on effective therapeutic techniques and at-home exercise programs to support continuous progress beyond scheduled sessions.
• Work closely with nursing staff, occupational therapists, and wellness teams to create and revise individualized care plans as necessary, ensuring a holistic approach to resident care.
• Maintain meticulous documentation in alignment with Medicare and state guidelines to ensure compliance and facilitate a continuum of care.
• Promote a positive atmosphere that cultivates dignity and nurtures the sense of community among residents.

Patient Population / Client Focus

Your primary focus will be on serving geriatric clients in a home care environment, where you will assist in overcoming the specific mobility challenges they face due to age-related conditions, post-surgery recovery, or chronic illnesses. This role allows you to form deep relationships with clients and their families, making you an integral part of their health journey.

Qualifications

• Active and valid Physical Therapist license in the state of Virginia (or eligibility for licensure upon hire).
• Bachelor's Degree in Physical Therapy from an accredited institution, with a Doctor of Physical Therapy (DPT) degree preferred.
• Current CPR/BLS certification or a commitment to obtain it at the time of employment.
• Demonstrated ability to communicate effectively and empathetically with patients, families, and interdisciplinary team members.
• Strong organizational skills, with the capability to manage a varied caseload and meet deadlines.

Preferred Experience

• Two years of professional experience in physical therapy, particularly within geriatric or home care settings.
• Experience working collaboratively in a multidisciplinary team environment.
• Familiarity with mobility aids and adaptive therapeutic equipment.

Compensation and Benefits

We offer a competitive annual salary ranging from $90,000 to $100,000, supplemented by a comprehensive benefits package that includes medical, dental, and vision coverage, along with a 401(k) plan.
