Occupational Therapist OT

We seek a highly skilled and experienced Occupational Therapist to provide home health therapy services within assigned counties including Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville. The Occupational Therapist will deliver patient-centered care involving evaluation, treatment, and re-certification visits, ensuring compliance with clinical and documentation standards. This role requires strong communication, flexibility to travel within and occasionally outside the service area, and willingness to treat Covid+ and PUI patients. New graduates are welcome to apply. Responsibilities:

• Conduct patient evaluations, re-certifications, routine visits, and discharge planning.
• Provide skilled occupational therapy treatment in home health settings.
• Complete all required documentation including OASIS assessments and progress notes.
• Collaborate with interdisciplinary teams, patients, and caregivers to optimize care plans.
• Manage travel and scheduling within assigned territory and occasionally outside the service area as needed.
• Maintain productivity standards by achieving required weekly points through patient visits.
• Adhere to infection control policies including treatment of Covid+ and PUI patients.
• Communicate effectively with patients, providers, and agency staff regarding patient care and scheduling.

• Current California Occupational Therapist license.
• Basic Life Support (BLS) certification.
• Valid driver's license, automobile insurance, and vehicle registration.
• Up-to-date immunizations and screenings including TB (annual), MMR, Varicella, Hepatitis B, TDap (within 10 years), Flu (annual), drug screen, and physical exam.
• Ability to meet productivity requirements of at least 27 points per week or 36 guaranteed hours if applicable.
• Minimum 1 year of experience preferred; new graduates considered.

• Home health experience.
• Experience working with a diverse patient population in outpatient or home health settings.
• Strong time management and organizational skills to manage travel and documentation efficiently.

• Must be willing to travel within assigned counties and occasionally outside the service area.
• Weekend coverage may be required, especially for non-guaranteed hour assignments.
• All requested time off (RTO) must be disclosed at submission; post-acceptance RTO requests are not approved.
• Ability to treat Covid+ and Persons Under Investigation (PUI) patients is mandatory.
• Compliance with agency contract terms including identical start and end dates is required.

• Options for guaranteed or non-guaranteed hours.
• If guaranteed, a minimum of 36 hours per week with at least 27 productivity points is expected.
• Non-guaranteed hours assignments require flexibility with no minimum hours guaranteed.
• Shifts are determined based on patient visit scheduling and may include weekends.
• Productivity and hours are calculated based on visit types and points system, with all-inclusive time covering travel, visits, documentation, and communications.
